The information on this Site may be changed by ABPT without notice. ABPT may change or withdraw any products or services described at any time without notice.

By accepting these terms of use, you will be allowed access to programs, reports and information protected by federal and state law contained in the secure portion of this website. Only authorized persons in lawful possession of a password provided by ABPT to provide such passwords may enter and access the secure portion of this website. The use of this website is subject to the terms of use contained herein.

Once you have accepted the terms of use, you will not be asked to accept such terms again when you access the site another time. ABPT has the right at any time to change or modify the terms of use that will be posted on this website. Any use of the website by you after modified terms have been posted will be deemed to constitute acceptance by you of the modified terms. ABPT has the right at any time to change or discontinue any aspect or feature of this website and to terminate any user’s access to the website. ABPT has the right, but not the obligation, to monitor your use and access of this site.

ABPT PROVIDES BOTH THERAPY AND WELLNESS SERVICES.

A PHYSICIAN MUST MAKE A REFERRAL FOR PHYSICAL THERAPY SERVICES AFTER PHYSICAL THERAPY HAS SURPASSED TEN CONSECUTIVE BUSINESS DAYS WITHOUT A REFERRAL.

WELLNESS SERVICES PROVIDED DO NOT COUNT AS THERAPY SERVICES.

Rates & Payment

· $180 per evaluation

· $110 per treatment for a single treatment

· $100 per treatment if purchased as a package

· The patient must consent to the agreed rate of services prior to evaluation/treatments.

· The patient must pay for treatment or set up a payment plan prior to each visit.

· A superbill (to self-file with insurance) is available upon request.

Cancellations/Refunds

· Cancellation fee: $50

o Patient will be allowed to cancel and reschedule >24 hours prior to treatment.

o Patient will be charged $50 if they cancel and/or reschedule <24 hours prior to treatment.

· ABPT will not treat if the patient or parent has a fever.

o Patient or parent must be fever free for 24 hours.

o If parent fails to notify ABPT of sickness prior to the appointment, ABPT will not treat the patient and the session will be considered a no-show (full charge)

· ABPT will fully charge the patient without reimbursement for any no-show visit. Examples of no-show visits: 1. Patient not showing up for an appointment 2. Patient not available scheduled home appointment.

· Refunds will be given at the discretion of ABPT.

Attendance Policy

· If the patient has less than 50% recommended attendance by ABPT, the patient will be discharged from physical therapy services.

· If the patient has three last-minute cancellations or no-shows, the patient will be discharged from physical therapy services.

· Patient must confirm appointments within 24 hours of appointment.

Expectations of On-Site Visits

· The on-site visit radius is 15 miles from Belton, Texas 76513

o A $5 fee will be applied to the bill for every 10 miles over the visit radius

o ABPT may decline treatment if is too far outside of the visit radius.

· Parent(s) must give undivided attention to the treatment session. Distractions must be kept to a minimum.

· ALL consent and HIPAA forms must be signed prior to evaluation/treatment.

HIPAA Privacy

You have requested online access to confidential patient data that is protected by federal and state privacy laws. We take very seriously our obligation to protect this confidential patient data from unauthorized use or disclosure. Accordingly, we allow only those persons with appropriate authorization to access or provide confidential patient data through this website. Any dissemination or distribution of the confidential patient data contained in this website to anyone other than the authorized recipient is unauthorized and strictly prohibited by law. The privacy and security of confidential patient data is governed by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its enabling regulations.

During the process to activate your account, you agree to give us true, accurate and complete information about you, and to promptly update this information when it changes. If you do not update it, we may suspend or terminate your use of the website as explained below. Any personal information that you provide to us is subject to the terms of our policy on privacy.

When you activate your account, you will be assigned a user name and you will select a password so that you can access your account with us. You agree that you will keep this information confidential.
